UW PRIVACY<\/p>

Voor een goede behandeling is het noodzakelijk dat ik, als uw behandelende therapeut, een dossier\u00a0aanleg. Dit is ook een wettelijke plicht opgelegd door de WGBO. Uw dossier bevat aantekeningen\u00a0over uw gezondheidstoestand en gegevens over de uitgevoerde onderzoeken en behandelingen.\u00a0Ook worden in het dossier gegevens opgenomen die voor uw behandeling noodzakelijk zijn en die ik,\u00a0na uw expliciete toestemming, heb opgevraagd bij een andere zorgverlener bijvoorbeeld bij de\u00a0huisarts.<\/p>

Ik doe mijn best om uw privacy te waarborgen. Dit betekent onder meer dat ik:<\/p>

  • \u00a0zorgvuldig omga met uw persoonlijke en medische gegevens.<\/span><\/li>
  • \u00a0er voor zorg dat onbevoegden geen toegang hebben tot uw gegevens.<\/li><\/ul>

    Als uw behandelende masseur\/therapeut heb ik als enige toegang tot de gegevens in uw dossier.\u00a0Ik heb een wettelijke geheimhoudingsplicht (beroepsgeheim).

    De gegevens uit uw dossier kunnen ook nog voor de volgende doelen gebruikt worden:<\/span><\/p>

    • Om andere zorgverleners te informeren, bijvoorbeeld als de therapie is afgerond of bij een verwijzing naar een andere behandelaar. Dit gebeurt alleen met uw expliciete toestemming.<\/li>
    • of voor het geanonimiseerde gebruik tijdens intercollegiale toetsing.<\/li>
    • Een klein deel van de gegevens uit uw dossier wordt gebruikt voor de financi\u00eble administratie,\u00a0zodat ik of mijn administrateur, een factuur kan opstellen.

      Als ik vanwege een andere reden gebruik wil maken van uw gegevens, dan zal ik u eerst informeren\u00a0en expliciet uw toestemming vragen.<\/p>

      Deze gegevens in het cli\u00ebntendossier blijven zoals in de wet op de\u00a0 behandelovereenkomst wordt\u00a0vereist 15 jaar bewaard.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t
